Consent to Processing of Unique Identification Information by Telecommunications Providers

I hereby consent to the provision of my personal information by SK Telecom Co., Ltd. (hereinafter referred to as “Company”) to the third parties listed below in order to use the identity verification service (hereinafter referred to as “Service”) provided by the Company.
· 1. Recipients of Personal Information
- NICE Information Service Co., Ltd., SCI Information Service Inc.
· 2. Purpose of Use by the Recipients
- Generation of CI (Connecting Information) / DI (Duplicate Subscription Check Information) and provision to the Company
- Prevention of fraudulent use and handling of complaints
· 3. Personal Information Provided
- Resident registration number or alien registration number held by the Company
· 4. Retention and Usage Period by the Recipients
- 6 months after CI/DI generation
· 5. If you do not consent to the provision of the above personal information, you will not be able to use the service.

KT Corporation (hereinafter referred to as “Identity Verification Institution”) must obtain the user’s consent to use or provide to third parties the unique identification information collected from users in relation to the mobile phone identity verification service provided through Korea Credit Bureau (hereinafter referred to as “Company”) pursuant to the “Act on Promotion of Information and Communications Network Utilization and Information Protection, etc.” (hereinafter “Information and Communications Network Act”).
■ Article 1 [Consent to Processing of Unique Identification Information]
The “Identity Verification Institution” processes unique identification information to provide mobile phone identity verification services that allow users to verify their identity online without inputting a resident registration number, pursuant to Article 23-2(2) of the Information and Communications Network Act.
■ Article 2 [Consent to Provision of Unique Identification Information]
Pursuant to the “Standards for Designation of Identity Verification Institutions” (as notified by the Korea Communications Commission), I agree to the provision of my unique identification information to “other identity verification institutions” for CI/DI generation and duplicate subscription checks in connection with services contracted by the “Company”.

· 6. If you do not consent to the above, you will not be able to use the service.

I hereby consent to the provision and processing of unique identification information by LG Uplus Corp. (hereinafter “Company”) to the following third parties for the purpose of providing the mobile phone identity verification service (hereinafter “Service”).
· 1. Recipients of Unique Identification Information
- NICE Information Service Co., Ltd., SCI Information Service Inc.
· 2. Purpose of Use by the Recipients
- Generation of CI (Connecting Information) and DI (Duplicate Subscription Check Information) and provision to the Company
- Prevention of fraudulent use and handling of complaints
· 3. Unique Identification Information Provided
Resident registration number or alien registration number held by the Company
· 4. Retention and Usage Period
6 months after CI/DI generation
· 5. If you do not consent to the above, you will not be able to use the service.

In relation to the mobile phone identity verification service provided by SK Telecom Co., Ltd., KT Corporation, and LG Uplus Corp. (collectively “Telecommunications Companies”) and entrusted to Korea Credit Bureau Co., Ltd. (hereinafter “Company”), the Company must obtain the user’s prior consent to collect, use, and provide personal information including unique identification information pursuant to Articles 17, 22, and 24 of the Personal Information Protection Act and Article 24-2 of the Information and Communications Network Act.
□ Purpose of Collection, Use, and Provision
- To use unique identification information for providing mobile phone identity verification services that allow identity verification online without entering a resident registration number, pursuant to Article 23-2(2) of the Information and Communications Network Act
- To allow other identity verification institutions to process the following unique identification information if required for CI/DI generation and duplicate subscription checks related to services contracted by telecommunications service providers

- Resident registration number (for Korean nationals)
- Alien registration number (for foreign residents)
□ Retention and Usage Period
Unique identification information will be promptly discarded once the purpose of collection, use, and provision has been fulfilled.
However, where retention is required under applicable laws such as the Act on Consumer Protection in Electronic Commerce, the following retention periods shall apply:
- Records related to contracts or withdrawal of offers: 5 years
- Records related to payment and supply of goods/services: 5 years
- Records related to consumer complaints or dispute handling: 3 years
- Any other period permitted under relevant laws and regulations
□ Refusal and Consequences of Refusal
You may refuse to consent to the collection, use, and provision of unique identification information, and you may also withdraw your consent at any time. However, refusal or withdrawal may result in restrictions on service use.
